The Registered Nurse-Care Coordinator is directly responsible to the Nurse Manager and Chief Nurse of Specialty Care.
The RN Care Care Coordinator works in partnership with assigned specialty members and a panel of patients for effective disease control and chronic care management.
The applicant is responsible for coordinating and managing the care of patients with the diagnosis of HCV and/or HIV.
The applicant will utilize advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making and evaluating outcomes as well as evaluate current practice and formulate outcomes for groups of patients and/or organization processes with the area of expertise.
DUTIES:
Responsible for collaborating with internal and external services to facilitate care transition in order to effectively meet the patient's needs Provides patient and family health education with a focus on self-management, prevention, and wellness, based on patient's goals Collaborate with the Specialty Teamlet in developing the patient-driven holistic care plan for life Demonstration of a high level of personal and professional accountability and the ability to be self-directed Ability to problem solve and apply critical thinking skills, and resolve conflict Excellent interpersonal relations:
excellent assessment skills, good interpersonal, organizational, and leadership skills in previous and current position Ability to effectively communicate and coordinate patient care with the interdisciplinary team.
Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
