Clinical Pharmacist

Major duties include, but are not limited to, the following:
Fosters productive and cooperative working relationships by showing understanding, courtesy, tact, and politeness to others Regularly cooperates with coworkers and others in meeting commitments and accomplishing assigned work on time Obtains medication histories from patients, provision of patient instruction regarding the safe and appropriate use of drugs, and documents findings in the medical record. Monitor drug therapy regimens, including routine analysis of patients' drug therapy, medical problems, laboratory data, and specialized procedures, and correlates these analyses with others responsible for the patient's care as warranted. Maintains good workflow patterns to insure the provision of prompt, accurate drug distribution services to patients. Reviews prescription orders for completeness and adherence to Medical Center Policy, VHAs directives, and Federal Regulations. Reviews data for correct patient, medications, strength, directions, quantity, day's supply and number of refills. Screens for allergies, duplicate therapy, and Drug-Drug interactions. Assures that all information is correct prior to finishing the prescription for dispensing. Maintains good workflow patterns to insure the provision of prompt, accurate drug distribution services to inpatients, units and clinics. Reviews prescription orders for completeness and adherence to Medical Center Policy, VHAs directives, and Federal Regulations. Reviews data for correct patient, medications, strength, directions, screening orders for drug-drug and drug-food interactions, dosing errors, allergies, incompatibilities, contraindications, and formulary availability. Working knowledge of the operation and basic daily maintenance of all automated equipment in the pharmacy (e.g. Omnicell, EXP, ScriptPro, Q-matic). Knowledgeable about contingency plans and all departmental SOPs for automation. Responsible for an advanced knowledge and daily application of Joint Commission Standards on Medication Management to ensure compliance and continuous readiness for any survey; Is knowledgeable on patient safety issues and know when to notify supervisors if they identify any patient safety issue that may arise. Checks re-labeled prepackaged large and small volume parenterals for inpatient use. Has mastered aseptic technique and has completed initial competencies and all testing required by USP 797 standards. Assesses each compound and pulls all necessary items needed to compound product. Checks all compounded items for appropriate calculations necessary for compounding product prior to technician mixing. Prepares and fills controlled substances as required for filling of inpatient/outpatient orders and Omnicell. Follows VHA controlled substance handbook 1108.01/02 for processing all controlled substances through the vault. Regularly complies with agency, federal or other pertinent regulations, policies and procedures as related to controlled substances. Maintains an effective liaison with staff members of all professional disciplines and services for consultation regarding medications, pharmacotherapeutic problems, and relevant pharmacy operating policy and procedures and pharmacy administration to modify procedures when necessary. Functions as second-line telephone contact for patients, providers and/or the public. When necessary, addresses telephone requests in a timely and courteous fashion. Forwards calls beyond the pharmacist's scope to a clinical pharmacy specialist or supervisor. Employees are typically expected to work forty hours per week. This may include nights, weekends and/or holidays as assigned. Principle duties require repetitive use of hands and fingers (e.g., preparing IV admixtures, use of a computer keyboard, etc.). May require lifting and carrying light loads (up to 44lbs.), including boxes, equipment, unit dose-cassettes, and IV solutions; stooping or kneeling (e.g., to pick up items from the floor, remove and replace items on lower shelves, and to file documents in lower file drawers, etc.); sitting, walking, or standing for long periods of time (4-8 hours). Normal ranges of hearing and vision are required to perform work and communicate effectively; must have the visual ability to distinguish between basic colors and shades of color. An ability to perform tasks requiring rapid mental and muscular coordination functions simultaneously is necessary. Must be physically capable of operating various types of equipment used for the job. Contacts are with co-workers in the pharmacy, medical staff, nursing staff, and other health care providers at the facility, research investigators, pharmaceutical vendors, patients and the public. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. years of Clinical Pharmacist experience. Grade Determinations:
GS-12 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level which demonstrates professional and administrative management of an assigned area in pharmacy service, to include budgetary execution. Assignments at the GS-12 grade level include:
handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to:
reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Must fully meet the following KSAs:
(Knowledge of professional pharmacy practice; Ability to communicate orally and in writing to both patients and health care staff; Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security; Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters). Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
